SECOND ANNUAL NFC SOUTHEAST KICKBOXING CHAMPIONSHIP FIGHTER CHECKLIST
All fighters must complete the following requirements in advance of the Second Annual NFC Kickboxing tournament.
Please review carefully and complete each step as soon as possible.
To make this as seamless as possible, we are also attaching our spreadsheet that shows current results of what we have from each fighter so that you can see if we’ve received your Fighter Application & Release, Blood Test, Eye Test, Physical and weighin results. We update this many times during the day. You can access the spreadsheet by clicking HERE (link will be added soon)
1. ISKA Fighter Application & Release
All fighters must complete and sign the online ISKA Fighter Application & Release online form. This is free to do and will only take you 5 minutes. Just click the link embedded above and it’ll pop up in a new window to fill out.
2. Blood Test (Hepatitis B, Hepatitis C, HIV)
You must turn in a Blood Test that is current within 180 days of September 26 if you are 18 or older on fight day. If you already have one, please send it to davidoblas@nfcfighting.com
We recommend using our partner clinic, EnFuze Life, which offers a special NFC rate of $60 (compared to $99+ elsewhere).

Be sure to do this in advance as results typically take 2-3 days, so please do not wait until the last minute.
This clinic will automatically email results directly to the NFC.
This is only required for fighters 18 and up.
If you use a different clinic, please forward your results to davidoblas@nfcfighting.com
3. Eye Exam
All fighters (18 and older on fight day) must submit a completed eye exam form.
You can complete this exam at your local Target (if they have an eye clinic) or any eye clinic. Many times you’ll need to go online or call to book a time slot.
This official form must be used.
Once complete, take a clear photo of the signed form and email it to davidoblas@nfcfighting.com
4. Physical Examination
A physical exam is required using this official ISKA form. If you have done a physical in the past 12 months and it states that you are fit to compete in a kickboxing match, you can email it to davidoblas@nfcfighting.com
We will have a doctor at the September 26 tournament that can do these physicals. But with 250+ fighters competing, we are hoping to get many of these physicals done in advance so fight day can be run smoother. PLEASE PLEASE PLEASE do everything you can to get this done ahead of time. It will just make your fight day run much smoother.
This official ISKA form must be used if you can get it done in advance.
Once complete, take a clear photo of the signed form and email it to davidoblas@nfcfighting.com
5. Fighters Age 37 and Older
If you are 37 or older, additional medical requirements are needed per ISKA regulations.

6. Weighins on Friday, September 25 via Facebook LIVE
